Job Description
Each Practitioner will be responsible to complete a medical assessment in a home environment. Assessment shall mean a medical examination to obtain a comprehensive medical assessment of Members (Patient) for the purpose of facilitating the Program’s disease management, care management, case management and medical therapy management services. The qualified Practitioner will ensure the patients within this program have a clinical evaluation that can be used in communicating with the Member’s physicians concerning the Member’s medical condition and their personal medical needs documented in a way that captures the Member’s condition in a timely, accurate and complete way.
